Allowable Requests for Assistive Technology Equipment
The federal definition of assistive technology is:
"Any piece of equipment, or product system, whether acquired commercially off the shelf, modified, or customized, that is used to increase, maintain or improve functional capabilities of individuals with disabilities."

Financial Need Requirement
By signing/certifying the Bellows Program application (text, PDF), the UCP affiliate executive director assures that the UCP affiliate "deems" the Bellows Fellows recipient is "in financial need."
By signing/certifying the applications, the UCP affiliate executive director assures UCP that all elements of the application submitted are accurate. Electronic submissions need to come from the e-mail address of the executive director with a brief narrative supporting the application.
